*The PGA requires that a signed Facility Authorization Agreement be on file annually at The PGA in order for
your Golf Retirement Plus^™ incentives to be deposited into your retirement account.

*West Coast Trends, Inc/ Scheyden Eyewear requires that your opening order be a minimum of 24 frames of your choice. This will also be the minimum annually to qualify. Only sales paid within terms qualify towards GRP incentives.
